(b) Provide a critical analysis of the monitoring process with reference to the video case study (c) Provide a transition plan for the client following the case manager’s move to another position. In your assessment you need to consider potential obstacles in case management practice.
Tasks: 1. Using the same video case study from assessment 1. Watch the following: Case management case study video 2. Using the video case study, the textbook, subject notes and your own research provide a critical analysis of: (b) The case manager’s role in the implementation phase including applying case management principles. (c) The monitoring process This assessment requires you to undertake a critical assessment of the case manager’s role and the monitoring process with evidence from readings that support your argument. You might analyse what works, what doesn’t work, gaps, what could be done differently, evidence-based practice etc in relation to case management practice and meeting the needs of the client. This is not just a description of what happened in the video. You need to develop a thesis or argument regarding case management practice in this case study.
In your assessment you need to consider potential obstacles in case management practice. 3. Develop a transition plan for the client to move to a new case manager for the client The case manager is taking on a new role in the organisation and will no longer be the client’s case manager. The transition plan needs to take into account the needs of the client, the new case manager and the other services supporting the client. 4. Ensure you correctly cite references in the body of the assessment and provide a reference list of all your sources including the textbook and the subject resources. It is essential that you take the time to accurately reference your work to avoid plagiarism. Plagiarism, even if unintentional can result in a failed assessment.
